New Patients
As a new patient, you will be introduced to our practice through a comprehensive examination performed by Dr. Truong. We want to get to know you, your dental history and your concerns and expectations.
To save time at our office, you may complete the 'New Patient form' below.
If you have dental insurance, bring the necessary proof of insurance (dental insurance card). If no card is available, Social Security number of insurance holder may be required.
Please alert us to any medical conditions that may be of concern prior to treatment.
